1. Download the passport application form on this site and complete all the entries to avoid unnecessary delay. For minor applicants, a parent may accomplish the form on their behalf. Those who are either applying by mail or are not applying in person, the duly-accomplished application form must be notarized by a local Notary Public before submitting the application to the Consulate.
2. The applicant's latest passport must be submitted for cancellation. A photocopy of the data page showing the applicant's picture and personal details, including the latest immigration arrival stamp by the U.S. Immigration and Naturalization Service (INS) must also be submitted for the Consulate's records.
3. Submit proof of the applicant's actual residence, e.g. driver's license, State ID, Employment ID, or other similar documents evidencing actual residence.
4. Submit three (3) identical passport-sized photos - 1.77" x 1.37" (3.5 x 4.5 cm)- taken within the last six (6) months. The photo must show the applicant's front view, clad in decent attire (not in sleeveless attire) and without eyeglasses. Digital photographs will not be accepted.
5. In the case of applicants who are holding brown-colored passports, documentary proof of middle name (e.g. birth certificate, marriage certificate, social security card, etc.) must be submitted. The applicant should spell-out his or her middle name (not middle initial) as this will be reflected in the new passport. Holders of green-colored passports are not required to submit documentary proofs of middle unless the consular officer requires the submission of such documents.
6. For applicants who are military servicemen/military personnel and who came to the U.S. using only military ID and without a Philippine passport, a Certification (with duplicate) issued by his/her Commanding Officer to the effect that he/she is still a Filipino citizen who is on active duty with the U.S. military service must be submitted. In addition, applicants falling under this category must submit:
a) a duly accomplished application form;
b) three (3) passport-sized photos;
c) original or authenticated copy of birth certificate issued by the National Statistics Office; and
d) other supporting documents which will establish the applicant's identity as a Filipino citizen, i.e. marriage certificate, school records, professional ID, etc.
7. For married women wishing to use their married name -- see item C on Amendment of Passport. Finally, it is important to bring the original copies of the supporting documents required for a passport application to avoid delays in the processing of application. The original copies may need to be presented to the Consular Officer for verification purposes. These will be returned to the applicant after the verification process is completed. It is also important to bring additional photocopies of all the documentary requirements to ensure that sufficient copies are available should these be required.

In order to clearly establish applicant's personal details, the original or authenticated copy of the Birth Certificate issued by the National Statistics Office (NSO) must be submitted in support of the application. NSO-certified civil registry records may be obtained through the following methods: (a) In Person at: the National Statistics Office, East Avenue (in front of SSS), Quezon City or at the Office of the Civil Registrar General, EDSA corner Times St.West Triangle, Quezon City 1104,Philippines; (b) By Postal Service System: address requests for certification to the Civil Registrar General, National Statistics Office, P.O. Box 779, Manila, Philippines; or By E-mail: send the details of the application to: L.Hufana@mail.census.gov.ph. In the alternative, the NSO also processes applications for certification online through its website www.e-census.com.ph. If the birth registry record of the applicant is lost or not available, the following documents must be submitted: a. Certificate of Non-Availability of Birth Record from the Local Civil Registrar of the applicant's place of birth. b. Original/certified true copy of the baptismal certificate, voter's ID or any other public document indicating birth particulars and Philippine citizenship. c. Joint Birth Affidavit by two disinterested persons concerning the applicant's birth particulars.
5. In addition to the documents referred to in item (4), the applicant must also submit other supporting documents which will establish his or her identity as a Filipino citizen, e.g. marriage certificate, school records, professional ID, or other similar documents.
6. To ascertain the fact of loss of the passport, an Affidavit of Loss of Passport, duly notarized and signed before a Consular Officer must be submitted. The applicant must state in the Affidavit the circumstances surrounding the loss of the passport, together with the details of the lost passport such as the passport number, date and place of issue, if known by the applicant. If the passport was stolen, a corresponding Police Report must be attached with the said Affidavit.
7. If available, a photocopy of the data page of the lost passport or other previously issued passport may be submitted. All those applying for the replacement of lost passports will be interviewed by a Consular Officer. Processing of Lost passport is subject to 15 working days waiting period from date of receipt of application. It is important to bring the original copies of the supporting documents required for a passport application to avoid delays in the processing of application. The original copies may need to be presented to the Consular Officer for verification purposes. These will be returned to the applicant after the verification process is completed. It is also important to bring additional photocopies of all the documentary requirements to ensure that sufficient copies are available should these be required.

1. In addition to the requirements for Renewal of Passport, the following documentary requirements must be submitted by a female applicant wishing to change her maiden surname to her married name: a. For a marriage solemnized in the Philippines - submit the original/certified true copy and a photocopy of marriage certificate. b. For a marriage solemnized outside of the Philippines and celebrated within the jurisdiction of the Philippine Consulate General(i.e. in Southern California, Southern Nevada, Arizona, New Mexico and Texas), the following must be submitted: 1) original and photocopy of marriage certificate; 2) duly accomplished Consulate Report of Marriage Contracted Abroad form. (Note: Registration of Marriage requires the following: original copy of the marriage contract/certificate/license, passport photos of both parties, proof of citizenship of both parties, i.e. passports or birth certificates. 2. For female applicants wishing to revert to the use of their maiden names: a. For widowed applicants - certified true copy and photocopy of death certificate of husband, or a court order of presumptive death. b. For annulled marriages - certified true copy and photocopy of annotated Marriage Contract or Certificate of Registration and certified true copy of Court Order. c. For divorced applicants (Allowed only in cases where the non-Filipino spouse acted as the plaintiff; and not allowed when both parties are Filipino citizens at the time of the divorce), submit the original copy of Divorce Decree. In all the foregoing cases, the female applicant must accomplish a Sworn Declaration to Revert to Maiden Surname. A sample form of this Affidavit can be downloaded in the Consular Services category.
1. The Personal Appearance of the first-time applicant is required.
2. In addition to requirements (1) and (4) for Renewal of Passports, the applicant must also submit the original/certified true copy and a photocopy of Birth Certificate from the National Statistics Office in Manila, or if not available, substitute the following (a, b, and c): a. Original/certified true copy of non-availability of birth certificate from the National Statistics Office (NSO) in Manila. b. Original/certified true copy of baptismal certificate or any other acceptable public documents. c. Joint birth affidavit by two disinterested persons.
3. For persons born in the U.S. of Filipino parent(s), in addition to requirements (1) and (4) for Renewal of Passports, the birth should first be registered with the Consulate by submitting the Report of Birth Form (downloadable form under Civil Registration) if the birth occured in Southern California, Southern Nevada, Arizona, New Mexico or Texas. Passport application by mail is accepted, provided the accomplished form is notarized by a duly authorized Notary Public in the locality where the applicant resides. For tracking purposes, please call (213) 639-0980 (ask to be connected to the Incoming/Outgoing Mail Section) for confirmation of receipt or delivery. The applicant can make arrangements for the return of the processed application. If the applicant wishes the Consulate to arrange for the return of documents, an additional fee of $6.00 must be paid to cover for mailing expenses. The Philippine Consulate General is not responsible for any delay or loss during transit by mail or by any courier services.
The modes of payment that are accepted by the Consulate are those made in cash (do not send cash if applying by mail), postal money order or cashier's check made payable to the Philippine Consulate General.
