Procedure for Reporting the Death of a Filipino Citizen

The death of a Filipino citizen abroad must be reported to the Philippine Embassy or Consulate exercising jurisdiction over the place of death to ensure that the occurrence of death is properly registered and recorded with the Office of the Civil Registrar General in Manila.

REQUIREMENTS

1. The Report of Death Form (download Form No. 39) must be completed and submitted, together with the following supporting documents:

Proof of Philippine citizenship of the deceased (i.e. Philippine passport);
A copy of the deceased’s Death Certificate;
A copy of the Permit of Disposition of Human Remains; and
A copy of the Mortuary Certificate

2. The fee for this service is $25.00 payable in cash, postal money order or cashier’s check made payable to the Philippine Consulate General. An additional fee of $6.00 will be required if the interested party wishes a copy of the Report to be returned by mail.
